Supporting Your Journey to Wellness
Outpatient Therapy
Licensed professionals provide individual and group therapy in our office or in the community, offering compassionate care tailored to your unique needs.
Case Management Services
Our case managers coordinate and connect you with resources to improve daily functioning and meet personal goals. Services include treatment planning, assistance with food, clothing, housing, transportation, Medicaid/SSI/SSDI applications, and referrals to specialized programs.
Peer Support Services
We match individuals in recovery with trained peers who have faced similar challenges. These mentors offer guidance to help you stay out of the hospital, build daily life skills, and successfully integrate into your community. Peer support is available one-on-one or in group settings.
Assertive Community Treatment (ACT)
This intensive, evidence-based program supports individuals needing extra help to remain in their communities. The ACT team provides 24/7 care and assistance year-round, offering proactive and flexible support whenever it’s needed.
Medication Management
Our medication management services provide ongoing clinical oversight to ensure psychiatric medications are safe, effective, and tailored to each individual’s needs. Licensed providers—including psychiatrists, nurse practitioners, and physician assistants—evaluate, prescribe, monitor, and adjust medications to support the treatment of mental health conditions. Through regular follow-up and personalized care, we help individuals achieve greater stability, symptom relief, and overall well-being.
