Assertive Community Treatment (ACT Program)

The Assertive Community Treatment (ACT) Program provides intensive, community-based mental health services for individuals living with severe and persistent mental illness, including major depression, schizophrenia, schizoaffective disorder, and bipolar disorder. Serving Wayne County residents, our interdisciplinary team delivers personalized care directly in the home and community to help reduce hospitalizations, improve medication adherence, and support long-term stability. With 24/7 emergency support available, ACT is designed to help individuals live more independently and thrive in their everyday lives.


Available ACT Services

Clinical & Psychiatric Care

Licensed providers, including psychiatrists and nurses, deliver ongoing evaluations, medication management, and clinical support to ensure effective treatment.

In-Home & Community Support

Services are provided in the home and community, helping individuals build daily living skills and maintain independence in familiar environments.

Housing & Employment Support

Assistance with accessing stable housing and supported employment opportunities to promote self-sufficiency and long-term success.

Substance Use Recovery Services

Integrated support for substance use disorder, helping individuals achieve and maintain recovery as part of their overall mental health care.
