Case management is a service that assists consumers in obtaining services and supports that are goal-oriented and individualized. Services include:

• Assessment
• Planning
• Linkage
• Advocacy
• Coordination and monitoring to support consumers in gaining access to needed health, education, social services, and natural supports.

These services are developed through the person-centered planning process and are available to high risk adults with chronic mental illness and those with co-occurring substance use disorders.
