Case Management
Case management is a service that assists consumers in obtaining services and supports that are goal-oriented and individualized. Services include: assessment, planning, linkage, advocacy, coordination and monitoring to support consumers in gaining access to needed health, education, social services, and natural supports. These services are developed through the person-centered planning process and are available to high risk older adults with chronic mental illness and those with co-occurring substance use disorders.
Skill Building Services
The goal of skill building is to assist individuals with developing the social, educational, and psychological skills necessary to live as independently as possible. Treatment is mainly focused on peer socialization and group support in a structured environment. Along with an ongoing assessment of the individual's level of functioning in the community, the program provides: individual and group therapy, a goal directed treatment plan, crisis intervention, therapeutic recreation, structured leisure activities, educational and life skills services, and opportunities for community integration.
The focus of the program includes:
- Out of Home adaptive skills training
- Assistance with acquisition, retention, or improvement in self-help, socialization, adaptive skills, and supports services.
Psychiatric Services
These services are designed to enable individuals with a chronic mental illness to remain in the community with stabilized symptoms. The services include yearly psychiatric evaluations, medication management, and monitoring for side effects of psychotropic medications.
Substance Abuse
These services aim to educate, prevent, and treat substance abuse. Treatment services include education for the individual, family, and caregivers. Individual and group counseling may be provided in the home or another convenient location.

ACT ( Assertive Community Treatment)
Assertive Community Treatment are highly specialized services for consumers with severe mental illness. The range of treatment and services is comprehensive and flexible. Interventions are carried out at the locations where problems occur and support is needed rather than in a hospital or clinic setting. Services are available 24 hours 7 days a week. The team of professionals is assertive in engaging individuals in treatment and monitoring their progress. There is no arbitrary time limit on receiving services.
