Outreach Support Services

Social workers (and nurses when needed) visit the home to assess the needs of the elder, to develop a plan for maintaining independence and to provide counseling. Assistance is given in obtaining benefits and community resources, such as Social Security, Medicare, Medicaid, Food Stamps, housing, housekeeping, chore services, transportation and any other unmet need. Accompanied transportation is provided to elders with limited mobility and inability to use other transportation services.
