Healthy Aging

Stop HIV/AIDS and Addiction through Prevention & Education (S.H.A.P.E.)
A four partner collaborative working together to promote healthy choices in minority women age 50 and older through outreach and education.

We offer:

  • Four sessions of Healthy Choices curriculum

  • Information and referrals

  • HIV/AIDS and Substance Abuse prevention

  • Free anonymous HIV/AIDS testing referrals

      Butzel Senior Center
      Services and activities for elders that encourage independence, empowerment and self-esteem. Health promotion such as health screening, educational speakers and exercise. Activities such as arts and crafts, discussion groups, and sewing. Services such as limited transportation, counseling, outreach support and referral.



    • Community Information and Referral
      Social worker assesses needs and links consumers to resources in the community.



    • Caregivers Education and Support Program
      Workshops on the physical and emotional demands of caring for an older person. Subject areas include information about aging, community resources and alternative living situations. A monthly "Caring for the Caregiver" support group is also available.



    • Grandparents Raising Grandchildren Support Services
      For elders raising their grandchildren, services include respite (trained volunteers to care for the children in the home), educational and support groups, assistance locating and obtaining information about community resources, and substance abuse education and treatment for any family member. The grandchildren may participate in periodic tutoring, cultural programs, recreational activities and prevention programs.



    • Health Promotion and Substance Abuse Prevention
      Health education, screening and prevention activities for elder groups. Prevention activities include information and presentations to elder groups on substance abuse and a variety of other related health issues.



    • Outreach Support Services
      Social workers (and nurses when needed) visit the home to assess the needs of the elder, to develop a plan for maintaining independence and to provide counseling. Assistance is given in obtaining benefits and community resources, such as Social Security, Medicare, Medicaid, Food Stamps, housing, housekeeping, chore services, transportation and any other unmet need. Accompanied transportation is provided to elders with limited mobility and inability to use other transportation services.



    • Respite Services
      Family situation and needs will be assessed. Several alternatives are available to obtain temporary relief for family members who are providing care to an elder relative.
