Case Management
Specialized person centered program for high risk older adults who have a chronic mental illness.  Specific services include:  Assessment and Evaluation, Treatment Planning using the person centered planning process, Linking and Coordinating to community services, and Advocacy.

Guardianship
When appointed by Probate Court, the Agency assumes legal responsibility to help persons remain as independent as possible in the most appropriate setting. Services include periodic monitoring visits, advocating for continued quality of life, managing finances, making medical decisions and reporting to the Court.

Psychiatric Services
These services are designed to enable individuals with a chronic mental illness to remain in the community with stabilized symptoms.  The services include yearly psychiatric evaluation, medication management, and monitoring for side effects of psychotropic medications.

Outpatient Counseling
Individual services that support consumer’s efforts to remain in the community and avoid hospitalization or nursing home placement.  The program stresses consumer responsibility in the treatment/recovery process.  Services are provided in the home or on site, they include assessment of need, psychiatric and case management evaluation, person centered treatment planning, medication management, linking and coordinating, advocacy, and counseling.

Skill Building Services
The goal of skill building is to assist individuals with developing the social, educational, and psychological skills necessary to live as independently as possible.  Treatment is mainly focused on peer socialization and group support in a structured environment.  Along with an ongoing assessment of the individual’s level of functioning in the community, the program provides individual and group therapy, a goal directed treatment plan, crisis intervention, therapeutic recreation, structured leisure activities, educational and life skills services, and opportunities for community integration.  Psychiatric evaluation and medication management are provided, as well.  Psychiatrist and nurse on site. Transportation is provided.

Substance Abuse Treatment
Individual, family and group treatment elders, their caregivers and community groups in understanding, preventing and treating substance abuse. Treatment services include assessment, education and individual and group counseling that may be provided in the home or other convenient location.

Assertive Community Treatment (ACT)

What is Assertive Community Treatment (ACT)? It is an intensive service delivery model for providing comprehensive community based treatment to persons with persistent mental illness. The objective of the ACT team is to reduce inpatient hospitalization by providing all needed services in a community setting.
