Nurse Health Navigator

General Role Description:
In support of the Agency's strategic plan, under supervision of the Care Transitions Director or Coordinator, candidate administers risk assessments and participates in the team initiatives to support patients in independently reducing their personal risk for hospital readmission.

Education and Experience

  • Degree in Nursing from an accredited School of Nursing
  • Current license as an RN for the State of Michigan
  • One (1) to two (2) years of experience working in Community Health, with demonstrated ability to work with persons who are at risk because of age, income, mental or physical health, education, coping ability or environment.
  • Michigan driver's license and clear driving record

Knowledge, Skills and Abilities

  • Knowledge of sciences, principles, procedures, techniques, trends and literature of nursing and gerontology.
  • Knowledge of the scope, activities and quality of care transitions work.
  • Demonstrated organizational skills and ability to work on multiple projects, achieve outcomes and meet or exceed expectations regarding timelines, quality and service.
  • Ability to analyze information, use sound judgement and make independent decisions in specific areas of responsibility.
  • Ability to apply analytical/critical thinking, keen insight and judgement in complex, fast-changing situations in order to effectively respond to changes, resistance, conflict, emergencies and complex, urgent situations.
  • Ability to observe, evaluate, accurately document and report facts, and meet strict reporting deadlines.
  • Demonstrate interpersonal communication skills to develop and maintain the confidence and cooperation of others; and to collaborate with staff, clients and other related groups on complex problems in order to take appropriate and effective actions and achieve results.
  • Ability to speak and write clearly, accurately and effectively for a range of audiences and purposes (reports, advocacy, in-services).
  • Ability to use Word, Excel, Outlook and Internet navigation at a high level with demonstrated ability to learn new computer software programs.
  • Ability to be flexible with changing and/or unpredictable work settings and assignments, work independently with minimal direction and to respond to written and oral direction.
