Duties:
Under the supervision of the Care Transitions Coordinator, the incumbent will:

• Administer an evidence-based practice tool to determine patient eligibility,
• Maintain updated and timely electronic documentation, and
• Participate in discharge meetings with multidisciplinary teams at hospitals.

Skills/Experience:

• Required: Effective and clear written and verbal communication skills
• Required: Organization and time management skills
• Required: Skilled in nursing assessments in a hospital
  environment
• Preferred: Experienced and comfortable working with
  multidisciplinary teams
• Preferred: Experienced in using electronic documentation
  systems
• Preferred: Experience working in hospital settings

Credentials:

• Required: Registered Nurse (RN)
• Required: Have own transportation, a valid Michigan driver's license and an acceptable driving record.
• Preferred: Bachelor of Science in Nursing (BSN)
