Family Support Group
Family members and caregivers are invited to join a monthly support group. Services are designed to:
  • Empower individuals and their families
  • Provide support
  • Assist with advocacy

Skill Building Services
These services are designed to improve the quality of life by developing skills to manage the tasks of daily living.

Social Skills development is enhanced through training in areas such as:

  • Communication and language
  • Human growth and development
  • Building healthy relationships
  • Self-advocacy and empowerment
  • Community integration
  • Social interaction
  • Art therapy

Skills building and activities of daily living are developed in areas such as:

  • Self care
  • Mobility training
  • Telling time
  • Money Management
  • Meal preparation
  • Vocational skills
  • Pre-Employment services
  • Referrals to employment services, as needed

Other Clinical Services offered include:

  • Occupational therapy
  • Rehabilitation therapy
  • Psychological testing
  • Psychiatric evaluation & medication monitoring
  • Nursing assessment & consultation

Respite
Several alternatives, such as in home assistance and community-based centers are available to obtain temporary relief for family members who are providing care to an individual with a developmental disability. For information on respite, ask your supports coordinator. Synergy has respite services available.

Supported Employment
Objectives of the program are to teach skills (done via skill building) find employment opportunities and provide support after employment. Job development is currently done via referrals to outside agencies.

Supports Coordination
Supports Coordination assists people and their families to access community services such as medical and dental care, transportation, and other resources. The primary goal is to assist individuals who benefit from being linked with community resources. Services Include:

  • Assessment and evaluation

  • Individual service plan development

  • Linking and coordination of necessary services

  • Follow-up monitoring

  • Advocacy

  • Assistance with obtaining guardianship

The Supports Coordinator works with community resources and other staff members to provide a community support systems. Family members and caregivers are invited to join a monthly support group.
